A Clinical Study of Colorectal Cancer



Abstract
This report included the clinical analysis and review of literature of 71 patients with colorectal cancer about incidence. location signs and symptoms, duratio, diagnostic method, operative procedure, stage, histologic type, macroscopic finding,
distant
metastasis, post-operative complications who were admitted and operated at the Department of General surgery, Seoul Adventist Hospital during the 5 years from January, 1988 to December, 1992.
1) The peak age incidence was 6th decade(31% of total cases),and prognosis according to the age was better in the older than the younger, and the sex ratio of male to female was 1.15:1.
2) The most frequent site f the tumor was the rectum(49.3%) and ascending colon(11.2%) being the next.
3) In the right colon the most frequent signs and symptoms were abdominal palpable mass, anemia, weight loss and in the left colon and rectum bloody tarry stool and bowel habit change were the most common.
4) The duration of symtptoms and signs prior to admission was the most commonly below 3 months(40.8%of total cases).
5) The diagnostic methods were digital examination, proctoscopy, sigmoidoscopy, colonofiberscopy, barium enerma and abdominal CT. In two cases explo-laparotomy was done.
6) Miles' operation was performed the most frequently in 20 cases(28.4%) and total resectability was 81.7%.
7) The staging of tumors were performed according to modified Dukes classification and Dukes C was the most frequent(34 cases; 47.9%). But Dukes B is Progressively increased and Dukes C is relatively decreased by early diagnosis and development
in
perception of cancer.
8) The most common histologic type was adenocarcinoma(97.2%).
9) The macroscopic pattern was most frequent in annular type(56.3%)
10) The most common distant metastasis site was pelvic organ(8 cases) and the liver(6 cases).
11) The most common post -operative complication was wound infection(14.1%),including urinary problem(5.6%) and intestinal obstruction obstruction(5.6%) and anastomotic leakage(4.2%)
